Onboarding Form

To get started, please fill out the short form below with your basic information. This is the first step in setting up your personalized dispatch profile. Once submitted, a member of our team will reach out within 24–48 hours to complete the onboarding process.

During that follow-up, we’ll guide you through submitting the required documents, including:

  • CDL and Driver’s License

  • DOT/MC Authority Letter

  • Certificate of Insurance 

  • EIN or SSN (for tax forms)

  • W-9 Form

  • Dispatcher–Carrier Agreement

  • Notice of Assignment (NOA) if factoring

  • Load preferences and driver availability hours

  • Factoring company details (if applicable)
